Update your contact info

All participants who qualify to complete the Year 2 survey will receive an email with their own unique survey link. If you have not received an email, please let us know as we may need to update your contact information.

We will need to know:

1. Your name (first and last);
2. Your date of birth (to validate it's you);
3. Your old email address (the one we sent last year's survey to); and
4. Your new email address (the one you want to change it to).
